Live Well with Diabetes Program

Diabetes is one of the leading causes of cardiovascular-related death in the U.S. and can lead to irreversible changes in vision, kidney function, circulation and the ability to fight infection. The Live Well with Diabetes Program focuses on diet and exercise, glucose monitoring, goal setting, sick day guidelines, communicating with family and healthcare providers and making small changes that improve health. What participants tell us, is not only are they better able to manage their own diabetes, they feel better and benefit from the support of others who are dealing with the same concerns. 

In 2016, the Southeastern Connecticut Health Improvement Collaborative identified diabetes, and specifically the underlying factors that lead to diabetes, as one of 3 priority health areas for the community to address. Ledge Light Health District, in partnership with Senior Resources, will facilitate three, FREE, six-week Live Well with Diabetes programs this year:

1. New London Program - Dates: March 6, 13, 20, 27, April 3, and 10

Time: 4:30am – 7:00pm

Location: New London Community Health Center. To register call Cindy at (860) 448-4882 ext. 302.

2. Waterford Program - Dates: April 18, 25, May 2, 9, 16, and 23

Time: 9:30am – 12:00pm

Location: Waterford Senior Center. To register call (860) 444-5839.

 3. Groton Program -  Dates: Sept. 7, 14, 21, 28, October 5, and 12

Time: 6:00 – 8:30pm

Location: Groton Senior Center. To register call (860) 441-6785. 

Physicians and pharmacists are encouraged to refer their patients. The program is open to adults of all ages - all are welcome. Pre-registration is required, so for more information or to register please contact Cindy Barry at 860-448-4882 ext. 302.
